But does it really do anything besides changing combustion sound?

Did they say "by 5%" or "up to 5%". If they can increase my mpg by 5%, I'll buy it. If it is "up to 5%" no thanx. There is a huge difference between the two. We have electric superchargers that increase power "by up up to 10%". IOW 0-10% means if you get nothing they are still legally correct.

Yes. No increase in mpg. Biodiesel gets the engine quieter but no mpg. Drop after B40.
