If you pulled the belt and ran it you've ruled out the tensioner and all accessory devices being the culprit. If you ran it at 50mph with the belt off you ought to have gotten a CEL......

Next I'd look at the motor mounts. You can use a floor jack and large rubber bushing and lift the bottom of the engine 1/4" to take the weight off the mounts and see if the noise disappears or measure them from below to see if they have collapsed. Dont lift more than 1/4" or YOU will damage the mounts.
