You might have a stuck plunger in the fuel distributor.

The first thing I would do is verify that the hoses going to and from the Idle Control Valve are secure at both ends. If they are, then I would remove the air cleaner and while someone is cranking the engine, slightly push down on the air flow sensor plate. This will allow fuel to spray out of the fuel injectors proving the pressures are in range.
