Crush washers ( in case this is not obvious ) are ONE TIME USE...
so NEW ones must be used each time it is ASSEMBLED
.
This even means that if you are working on it.. put it together... have to take it apart to add some other part or something... a NEW ONE must be used.... so it is very handy and good planning to buy some spares... in other words.. it requires a new one each time it is crushed... and being USED ( in service on the road ) is NOT the criteria...

I vote for clean threads, new crush washers, and proper torque being applied.
