Not a diesel but I almost lost a finger tip once bolting the fw back onto a VW air cooled (250+ftlb torque) and had my hand on the fly wheel when it slipped and tore a chunk out of my finger.
I always have dents and dings after working on the car but knock on wood, so far nothing major. I did learn a long time ago to always be wary of where my hand is going to go when it does (not if) slip (like when a bolt head breaks off or socket comes off bolt head etc).
Also learned a long time ago to never put head directly under parts I am working on.
