The rotors are worn and very likely need to be replaced. There is quite an impressive lip at the outside edge of the rotor. When new, the surface is completely flat all the way to the edge.

The pad wear does look a little uneven, but I wouldn't much worry about it. Given how long rear brake pads last it's just not worth worrying about.

Just replaced the pads & rotors and bleed the brakes - you'll be good to go for another 100K miles.
