Any suggestions on putting the box back on properly, or am I just blind and the splines are actually indexed to the steering arm?
if this is the pitman arm?

There is an alignment groove on the arm face and output shaft end...

If the steering wheel is not level:
Note:
Wheels straight, full vehicle weight on the tires.

#1. Remove the flex joint pinch bolt (at the gearbox).
#2. Insert a screwdriver in the pinch slot, and carefully drive it in to spread.
#3. (engine off), Grab the steering wheel and force it to center ignore the ratcheting feel/noise = slipping across the splines.
#4. If the screwdriver slips out to soon = you may need to repeat.
#5. Remove the screwdriver.
#6. Install and tighten the flex joint pinch bolt (at the gearbox).
#7. Test drive.
