what did you use the first time for an adhesive? I do believe the gear needs to be removed and the shaft roughed up then some loctite used to secure the gear to the shaft.

After various attempts to hold the gear in with epoxy (JB Weld), I ended up rigging a small mount to hold the gear in place better. It hasn't acted up since then.
