Just an idea but have you checked to see if the fan is hitting the underside of the top radiator hose? It sounds a little like the fan hitting something and I've found two hoses rubbed by the fan - one in my car and the other in a wreck I bought for the engine so it happens. If you find a cut in the the bottom of the hose then it's probably time for two new mounts...and a hose.
