This can be from rain or condensation and both those sources can be from your fuel supplier or could be introduced directly into your tank. If you have a car that has been setting it will be more of a problem.
A car that is driven regularly and suddenly has bacteria most likely got it from the fuel supplier.  |

It sounds to me like the car does not have a primary filter. The lift pump has been replaced but no filter seen.
