with another truck a long time ago. Assuming none of the other linkage issues solve the problem, the problem with mine was worn forks.

During a test drive, the Tx shop guy said "stop resting your hand on the shifter". Apparently, it's a common problem for all standard vehicles. In other words, once through the gears, don't leave your hand on the shifter. It will cause the forks to wear overtime.
