I am with you on this one Tom.
It looks like the caliper had jammed a long time ago & was only working on one side of the disk. The cast iron disk is no match for the steel backing plate. Like you, I have seen badly worn disk rotors on junk cars. Normally the rotors crack along the sides of the radial flutes & the wedge shaped pieces fall out. The brakes grab badly when this occurs. I saw a Celica with disks missing many pieces. Eventually the piston comes out too far & does not retract. This jams the rotor & stops the car suddenly.
On the W123 with solid disks, the disk can crack around the inner part that attaches to the hub & then no braking.
Those who doubt the pic probably just lack seeing extream situations like this.
