It sounds to me like the weather conditions changed after he took off.

What about those radios being on the wrong frequency? Is that something that the pilot routinely has to set each flight?

Its not that I cannot accept that he was fallible, I truly don't know about his skill level other than what is said in the report. I doubt you do either. I'm just trying to get at the truth of it using what is available to consider....without making anything up or reading his mind....posthumously.
Non-instrument rated pilots should not be flying in zero visibility. Night is zero visibility. He should not have been flying.

The radio worked just fine. When you fly GA you should monitor a specific freq (what is it Larry? 122.5?) and also monitor whatever airport is nearby so that you can keep up with traffic. I think when you're under instruments you are required to monitor the ATC freq. I expect that the pilot was engaged with his passengers rather than his airplane.
