IIRC, I set the parking brake and then used a small trolley jack to lift the breaker bar handle until it broke free. Then a simple ratchet finished removal.

When installing, I used longer "wheel" bolts for the hub and the handle for the floor jack to crush the spacer. It will likely take a very long cheater bar to get it to crush for the correct tolerance.
