Remove door panel, unbolt rear window channel, and remove the phillips screw up at the top. I think you will need to lower the rear window after you unbolt the channel, don't remember.

Glass (or what is left of it) will slide out with it's frame. Watch for little plastic pads that look like clips, they need to go back in the same place.

Insert new glass, re-install window channel, and replace door panel.
