What causes blow-by??

It think the general concensus is not to use additives.
Ok, here's another question: What causes the rings to not seal anymore? And how can I determine the cause?
Do rings simply wear out? Or are the cylinder walls damaged? Should I expect to have to machine the block? Are the cylinder walls glazed? If I can have this problem fixed by simply replacing the rings and honing the cylinder walls, then I'll definately do it this summer. Otherwise I'm going to live with the blow-by, as I'm not about to drop $1000+ on new pistons!
Where should I start?
