I found the location for the third bolt. There is a plastic sleeve over the housing part of the check (the part inside the door). The sleeve had slid back and was covering the threaded hole. I slid it forward with a sharp tool and the threaded hole for the third bolt was exposed. I have never seen this setup on an M-B before.
