99 e300dt loud idle, smokes at higher rpm

My e300 turbodiesel has developed a very loud idle. It also seems to smoke more than it used to at higher rpms. If I put the car in park and rev the motor to say 2000 rpm's (and hold it) the motor will begin to miss and smoke. It does not do this under a load. (Just possibly a puff of smoke.) The car seems to have great performance and no check engine light. I have also noticed when I decelerate the motor rpms will fluctuate about 6 times just before returning to an idle. (a rum-rum-rum-rum sound)

I repaired a fuel leak at the fuel cut off switch. I also have replaced all of the plastic fuel lines.

Any ideas?
