Your compression test kit might work for injectors or glow plugs, best to determine before you take parts out.

The OM60x engines are WAY easier to do the injectors for compression testing. The glow plugs are buried up underneath the intake manifold. Getting to them requires removal of the manifold, or some serious contortions.

Just a few questions:

1) What is the rpm at start, and after it begins shaking?

2) If you turn it of and restart, is the sequence repeated?

3) Is the delay always exactly 40 seconds?

It’s probably just worn injectors. The afterglow helps the engine run smooth for a short while and then fades out the help gradually. Even my sorted lowish mileage 602 does this very slightly. The less sorted the engine is the starker the transition from afterglow to unassisted cold running will be.

My 93 did the same thing. It seems common on the 124.128

Another question: if you run it hard for a while, does it still do this?

I gotta know if you ever fixed this.

I have at '92 that has the exact same issue. It starts fine, after 30 seconds there's a very rough idle and tons of smoke. Once it's warmed up, the idle is fine... but occasionally the turbo stops working until I restart it...?

I'm pretty sure I messaged you about this a year and a half ago and you hadn't figured it out yet, but I can't find the message to prove it.
