Dunno about a CAT but the park brake on the JCB 214 I have is completely separate from the road brakes.
I'd be willing to bet that 1) an operator runs with their foot on the brake(s) or 2) the brakes weren't adjusted properly (or possibly mis-assembled) from the outset and wore prematurely.
