I own a 2000 E320 automatic with 85 000 miles on it. When I take off, the gear sounds as if it needs to shift as the revs increase.The gear suddenly jerks violently ( jerks your neck) into the next gear. What causes this and is the transmission on its way our?

Checking the fluid level would be the 1st step.
Trans is computer controlled, if the fluid level is correct have the codes read
