300SDL door trim question

I've begun to do some rust repair on my 300SDL. Removed the rocker panel cladding and the door trim panels. When I removed the panels from the door, the rear most holder (not the red berry thing) was retained in the door. How do I get that out? Is it a screw or a bolt? Do I need to take the interor door panel off to access the thing?

I've sanded out most of the rust spots and will paint with Miracle Paint followed by a top coat of primer then try to match the Blueblack colour of the vehicle. There are a couple of places that will require some fibreglass patching, towards the front wheel well, but that is another topic.
