Tranmission Help

Hello, I just purchased an 1981 300D that I'm having some transmission problems with. When I first start the car when its cold and put it into gear nothing happens. The car just sits there, it doesn't matter what gear I try. If I rev the engine the car will start trying to move a little, then after about 5 to 10 minutes of revving the engine at about 2k rpms the transmission will jerk and go into gear. From the point the car will drive but has trouble shifting (it waits a long time to shift gears and sometimes I have to move the shiftier between 2 and S to get it to shift). It also slips sometimes while accelerating. I do know the car has a few vacuum issue (the car won't turn off with the key). Do you guys think this might be a vacuum problem, or is there something wrong with the transmission. The fluid level is full and nice and clean. Once the car gets going on the highway etc its fine. Any help would be great.
