I got it all apart and back to gether with a 1/8" washer and 1/16". Good thing I took it apart, lock washer was collapsed, and no woodruff key to be found in the unit. Replaced all, wirebrushed the housing, painted the pulley and fan, and looks a treat.

One cause of this is the use of belts that are too narrow. The belts wear and start to ride further down in the grooves causing slippage. Somebody will then tighten the alternator to fix the issue and it goes away for a while. After a few repeats the tension bolt gets bent...
I had this issue on the 300SD. Belts I got from the dealer were too narrow. After tightening them twice I went to a NAPA dealer and when he compared the belts I had to what he was going to sell me, the ones from the dealer were 2mm too narrow! Make sure you only use 15mm belts.
