I guess MBZ figured us 240 drivers would be smart enough to always stay over to the extreme right side of the road anyhow and not need a mirror on that side.

Seriously, I know where one is I think, are they hard to install?

Mercedes passenger-side mirrors were optional for many years. The cars do look a bit unbalanced without one.
Many years ago, I paid $80 for a new passenger mirror from Mercedes for my '72 250 sedan. They were an extremely rare option on those cars, and almost impossible to find in salvage-yards. Fortunately, the W123 mirrors are much more common.
I added passenger-mirrors on my first '82 240D and my recently-sold '81 240D. Paid $50 for the first one at Potomac German Auto, and $25 for the second from a Forum member.
It's pretty easy to add a manual mirror, though if you need to pull one at a salvage-yard, removing the little plastic adjuster-knob is a bit tricky. And the little molded-plastic nubs that hold the chrome-plastic inside mirror plate to the door may be weak or broken, so you may need to add some double-sided foam-tape or a dab of glue there.
My latest 240D came with the manual passenger-mirror and my Euro 300TD came with a power-operated passenger-mirror.

drilling involved or not? The car isn't here right now for me to look at but I'm thinking maybe it's just removing some trim that's there or something? I found a federal 123 at our local yard, remember it had both mirrors but didn't really pay attention to condition, I'm going to stop by there today and look at it...

There's no drilling involved in a 123. Simply remove the trim and the mirror bolts right in. In right hand drive markets it was the (US passenger side) door that had the drivers mirror in it and the left side of the car went without. Simply find the right side mirror on ebay, junkyard or wherever and install it. I did that with my 240D as soon as I could source a right side mirror.

^ I doubt it, remove the cover, a hole should be there. As mentioned those were optional, so I don't think MB would have two different passenger doors on the assembly line
