W123 bumpers

I was looking at the bumper clamping rails for replacement. Looking at photo I am not sure what holds them tight up against the bumper? I know they show the little bracket like tabs, but I thought there must be something else to hold them on the bumper face. If that is the case, why do they fail and come loose. I see some from time to time without the bumper protectors.
