As for god is great, perhaps his motivation was indeed Islamic terrorism. We don't know. But I saw a PBS show about an archaelogical dig in Egypt, and in it, some locals had to lift a large, heavy stone. Before heaving, they'd yell, "God is great". It's a very widely used expression, so IMO, it's not that telling.

as a VA doctor ( shrink ) an air condition med unit is as about as close as he would get, and even if they brought anyone to the unit he might never see them it would be in a different building, shrink office in the field would be set up like something you would see in the states with a regular doctor - jz

People yelling "God is great!" has significance...otherwise, why yell at all? Why not grunt like a pig or an ox?
