that said, i once had a Q45 that slipped in final gear... it could still be on the road today..but i decided to part it out a couple of years ago.. drove it for thousands of miles that way..
to fix it, it will have to be overhauled

Have you tried adjusting the vacuum modulator?

BB46, I have a 1989 190e 2.6 that would slip from second to third gear. I read a post on adjusting the modulator. I tried it and the slipping stopped immediately. You might want to try it also. Take the cap off of the modulator on the side of the transmission, use a long, thin screwdriver. Make the steel pin slip out so as to be able to spin the pin. Turn it two times clockwise. The post I read said if it was still slipping, turn it one additional time. If it still was slipping after the three turns then the only option was to rebuild the transmission. Mine worked after only two turns.

Try bowden cable too
I just had some success with an even easier adjustment. I adjusted the bowden cable to "achieve earlier shifts", as directed by "stevebfl", who is a Moderator on the Tech forum. Adjusting the modulator was also part of the recommended procedure, but I have had a significant improvement so far with just the bowden cable adjustment.
The bowden cable is easy to get to right near the throttle cable in the engine area. Simply turn the plastic adjustment bolt clockwise so that it goes in just a little. I adjusted mine in quarter turn increments and then would drive a little to see what had happened. The idea is to get earlier shifts at lower throttle, which Steve explained is at higher vacuum. I would like to try messing with the modulator a little too, although right now I really seem to be pretty good with just the Bowden cable adjustment. Now if I could dial in just a little more pressure in the transmission, with the modulator, then maybe I would be totally cured. I guess the bottom line is that it is a bit of trial and error with the bowden cable and modulator adjustments.

The modulator is on the side of the transmission. It will have a vacuum line attached to it.
If you will do a search you will find lots more info.

I believe that "mod to shift earlier" is on the intake manifold. a cable links the throttle to the transmission, basically turn that plastic knob to tighten it up ever so little and you get earlier shifts.
