300d ROUGH idle!!!

What would some of the potential causes of rough idle be in a 1982 300D turbo.

Engine is smooth at acceleration but when you slow down and stop at a traffic light the engine is rough! Looking at it with the hood up, it rocks from side to side!!

220k miles. Don't know much about the service on the car. Guessing just marginal. Assuming not much more than oil changes.
