This happens a lot more than you would think (just check the google results).
No one really explains why, but my guess is not using a low-range torque wrench. If you figure that 50% overtorqued is only 22 ft lbs, you can see how jsut a little more on a big clicker wrench can get you into the danger zone.
If you see the botoom of this handout, they recommend a wrench wher the needed torque is between 40-80% of the wrench's range.

Re-using the bolts may be another contributor: "It not permitted to reuse the bolts M7x84; these must always be replaced." (From WIS on the M113)
