Why is my '91 300CE starting out in 1st gear?

My car seems to start out most of the time in 1st gear - leads to a lurching, hesitant start and then a quick, hard shift in to 2nd - sometimes even causing the tires to slightly squeal. This car has the 722.359 transmission, so I know it's supposed to start in 2nd - what should I be checking?

Thanks,
Ryan

Physically sticking "kickdown" switch underneath the pedal. I hear that can cause this type of behavior.
