FYI: w210 owners like the myself and Op, this manifested itself as a loud vibration from the front end of the engine. I personally thought the tensioner shock was bad but once I got to it I noticed that there is no tension on the belt. I used a screwdriver to leverage the spring and low and behold the top of it came right out of the sleeve.
