Years ago, I had a lingering cough for about 6 months, I went to 3 to 5 different MD's (can't remember the exact number) and they all prescribed various cough remedies, antibiotics, steroid nasal sprays.... none of which worked... cough persisted. One of those doctors was Indian (asian) and suggested nasal rinse. I did that a few times and it stopped the cough within a week or 2 and I was cured. I do not buy the packets, just 12 oz of non iodized saline solution and let all of it run through the other nostril/ mouth cavity... which completely rinses the entire nasal passages. Just breath through the mouth, not the nose.

Recommend a product called Neti Pot for allergy, sinus, cold, and congestion problems. It is basically a little hand held tea pot that comes with a saline solution. Mix a pot and pour it through each nostril, holding the other, and allow it to flow out the mouth. Do both sides. It absolutely completely clears you out and, in my instance, helped my allergies and congestion dramatically. Maybe found at Walgreens, etc. Highly recommended.

^^^

Second the neti pot for congestion issues. The Texas Hill Country area is one of the worst places in the US for seasonal allergies, literally everyone has problems this time of year and early spring.

The neti pot is definitely something you have to learn and acclimate yourself to (very unnatural feeling), but nothing works better for clearing up congestion. Usually using it twice a day for 2-3 days is enough to clear you out enough to function normally. And it's cheap...usually under $2 for a kit at the drugstore.

Back on topic though, if it's just the startup that's rough and smoothes out relatively quickly, I would lean more towards glow plug problems or air in the fuel system. I'd expect water to cause problems the whole time the engine runs, not just at startup.

If no water in the fuel filter, you may have air in fuel. To rule it out, install a temporary clear return to tank hose. It should have no air bubbles in it at all times, running or not.
Any idea as to the ID of the return to tank hose?

5/16". Easiest way w/o needing hose clamps is get some 1/4" i.d. clear PVC https://www.lowes.com/pd/B-K-1-4-in-x-1-ft-PVC-Clear-Vinyl-Tubing/1000116025 , soak end in boiling water for 30 sec then stretch over the barb. It won't leak or blow off since pressure is low and the PVC will fit tightly over the barb. Remove it once you're done with the test.
