E420 Power Loss, Surges

I have a slight miss at idle and power loss that seems to go away at higher rpms. Occasionally, at idle in drive, the car sort of lurches a bit...with uneven idle.

Car has 44k miles, plugs look clean. Original caps, rotors.

Any suggestions what to check?
