Our '91 190E 2.3 will be different - your belt goes around more stuff. But, if it helps, I had to remove fan shroud, fan blade with attached clutch disk, power steering pulley, lower idler damper attachment nut, 19mm idler securing nut, and 13 mm adjusting nut and hardware. On ours, had to remove the idler pulley to access the upper attachment for the damper. The difficult part is getting all the pieces arranged with the new idler assy back in. A diverse vocabulary is helpful.

My notes from when I did my 300sl.

1991 300SL Fan Belt Tensioner Change.

Tools: Ratchet Socket set with extensions. 13mm and 12mm sockets.
10mm hex head socket male.
8mm open end wrench.
13mm open end wrench.

Remove inner fan shroud: Pull plastic pin and rotate, lift and remove.
Remove plastic radiator cover. Snaps in. Lift up.
Remove plastic radiator fan shroud: Snaps in. Lift straight up.
Remove plastic air cleaner induction tube.

13mm socket. Squeeze ( hold ) fan belt while breaking loose 3 bolts on water pump pulley.
13mm socket. Squeeze ( hold ) fan belt while breaking loose 3 bolts on power steering pump pulley.

Remove fan belt.

Remove 3 bolts from both pulleys and remove pulleys ( pull forward ).
Note: Both pulleys are identical although mounted in opposite directions.

8mm wrench. Carefully remove electronic shaft position sensor and position it out of the way.

12 mm socket. Remove bolt at lower end of shock absorber. ( Tensioner ).
13 mm socket. Remove bolt at top of shock absorber. ( Engine ).
Note: Different size holes in shock absorber…Small hole goes down and screws to tensioner.

13mm socket. Loosen the large tension rod adjustment bolt. It will remove later.

10mm hex socket. Remove tensioner mounting bolt.

3 13mm bolts hold tensioner retaining bracket. Remove top bolt which is threaded into engine. Remove bottom bolt which is held on from the back with a nut. Loosen the left bolt on engine
( behind fan ) about 2 turns; no need to completely remove. Lift bracket upward and maneuver tensioner and shock absorber out. Center tension rod adjustment bolt in bracket hole and push it downward and out. Recommend new adjuster rod, collar and bolt assembly. ( 103 200 02 36 )
Note: Rod has a reverse thread.

Remove plastic position indicator from front of old tensioner. Position it on new tensioner with arrow at the minimum ( most right ) position.

Re-assemble everything in reverse order, but leave center bolt of tensioner about 1 turn loose. Locate fan belt threading diagram in owners manual and re-install fan belt.

Adjust tensioner rod bolt to tighten up belt. Use judgment, don’t try for super taught belt. Lock down center tension bolt to hold tension. Run engine and check belt.

Replace radiator shrouds and air intake tube.

For the '91 there will be a few differences, and a few tips. These are for the m103 engine - I think the '91 124 still used that engine??

- The front radiator shroud (plastic) is held in place with two metal clips, just pull them off. Release the rear shroud first by rotating, then release and remove the front shroud upward, then remove the rear one.

- The fan can be removed with an 8mm or 5/16 allen - if you lock the pulley down with the little "fan bearing bracket holding tool". Do a search herein for the "L" shaped coathanger-looking tool and how to use it. You can easily make one. Really helps.

- yours will have a big 'ol bolt right in the center of the tensioner body, and I'll bet it takes an 18mm socket rather than an allen. Very hard to see, keep looking.

- Loosen the big bolt a bit, then turn the 13mm cap on the tensioner adjusting rod CCW to loosen it. It does have left threads, but loosens and tightens the belt lefty-loosey - righty-tighty like a normal bolt. Plays with your brain.

- The brace over the front of the tensioner body runs over to the fan bearing bracket and bolts on to the lower left bolt there. When replacing that bolt, you MUST clean it up and apply gasket sealer to it - it goes into the timing cover and will leak oil.

- Adjust the "righty-tighty" cap on the adjuster rod before final tightening the big 18mm center bolt. Be sure to get that tensioner rod on the back of the tensioner correctly - it can be put on wrong. Look it over closely.

- set the pointer arrow on the tensioner to zero when everything is in place - it just clicks around.

Everything else is pretty intuitive. Good instructions above, also probably a pictorial in the archives somewhere.

Just an addendum - if the tensioner is shot, pressure on the fan belt may not provide enough friction to loosen bolts on the power steering pump pulley. What worked for me was to use a large screwdriver between two of the bolts to hold the pulley while loosening the third. Repeat for the other two.
