For duplicate pics, I right click on the pic and select "open with paint". I then draw a small yellow line in bottom right hand corner (just arbitrary and an OCD thing I guess) and save the file/pic with changes.

If the image is somewhere on the web already (they usually are), I just link to that one. If I need to upload my own pic, I either upload it to flickr (if it's a good photo I want the world to see) or I'll upload it to my personal website and link to it. Flickr generates all the different photo sizes for me, so I just upload and then copy/paste the URL.

If you're on a Mac, you can have an AppleScript that resizes photos for you, so you can take a big photo and drag/drop it on the AppleScript and it spits out a smaller format image that would work to use as an attachment for this forum.
