That's a common issue with Detroit's that have sat a long time. First thing to do is remove the valve cover and make sure the control rack is free before starting the engine.

I love how the old man just calmly walks around the truck, sticks a board on the intake, then gets a rag and chokes it out... I wonder what he was doing in the truck the whole time it was running away? likely trying choke, or something in the cab to kill the motor...

They dont have GP's & dont have much compression. many need it to start properly & not belch white smoke for some time. I believe its even recommended in cold conditions. We have been using it here for over a decade on a couple of GM's in earth moving gear.
