All I have ever done is every coupla years run a quart of ATF through 3-4 fuel fillups. Always makes a noticable difference. to me.
edit: and fill new fuel filter with ATF at its replacement

Lubricity additives aren't really worth the time. Testing has shown that a 5% blend of biodiesel had better numbers than any commercially available lubricity additive. B5 really needs to become mandatory all over.
Water on the other hand is a different story!

At least around here, B5, B20, and even B100 was readily available ~15 years ago from multiple locations. A few years ago though, it all went away. There is no Biodiesel available anywhere nearby anymore.

My guess is that a lot of the tax credits and incentives that were involved in the production have since expired. We had different ones here in Canada (and particularly my province) weren't always a good a deal as they were in the lower 48, but I remember after the laws that we did have changed I'm pretty sure the commercially available stuff from the few pumps we did have just pretty much dried up overnight.
I don't know how much of the true cost at the pumps is reflected in the price of petro fuels, but I know that with Bio - without a solid/comparable production and distribution infrastructure in place it's still costly and needs tax credits to be competitive with the petro stuff. It's still out there and readily available, but it's really localized and limited to certain states/cities/regions. Too bad, it's basically a no brainer to put it in all diesel IMO.

We used to get B100 at a local station (Arizona Petroleum owned by Colorado Petroleum), not sure if they still have it. We used it in the winter but in the summer went back to dino as the difference in BTU was noticeable while running the AC. After fuel prices came down the B-100 was higher than dino and remained higher. We now use Safeway which is selling B-20 (in Tucson)
