That is total BS. The school should never have issued such a form in the first place unless it were totally confidential. It is this sort of thing for which the ACLU exists.

I imagine this came from an admiinistrator without being run by anybody. A lawyer would have put the kebosh on it in a heartbeat.

They have a "community patrolman" on duty there. I would not be surprised they offered it up or that it was derived from some Federal Grant program guidelines/requirements
