The design of the r4 shell needs a bracket to hold the shell on. From the factory there is a 13 mm bolt that holds the lineset bracket against the shell. Without this bolt, the shell is only held on by a small bent metal tab.
Many ac shops leave off the small bolt...

Even "non-flammable" refrigerants can burn in an accident, due to the oil mist that accompanies a puncture. Many produce poisonous gas as a by-product. Indeed, R-12 produces phosgene gas (used in WWI). In recent crash tests, M-B found the new HFO-1234yc refrigerant produced HF acid, which etched the windshield. It can do worse to your eyes and skin. M-B refused to use it and shipped new cars to France w/ R-134A, which were held up because France already outlawed the later. Search.

I am sooooo hoping for auto manufacturers to start using r744... Clean, atmospheric, CO2... Pressures not as high as r1234y... But still quite high... The tricky solid phase issue if it gets below -180F... Shuldnt be an issue...

R704 would be a killer refrigerant, except for the massive pressures needed for vaporization and phase change... Totally inert too... Just a bit rare though for mass liquid production... And a leak would be detected by your high voice...

The GM R4 compressor is built with a center aluminum casting then a steel tube slides over the OD , a o ring trapped between the body and tube on each side is used for sealing. Improper mounting leads to the tube sliding off the body, apparently only on MB cars as I've never seen this on a GM car

Overpressure and missing hardware is hardly the compressors fault, it's like overloading a car with weight, having a tire blow out then blaming the tire.

Not that I could afford a new M-B. I am just a lowly engineer and bottom-feeder.

I am way behind the times. The M-B tests on HFO-1234yf flammability were in 2012. It is very nasty when it burns:

No, everything stock.
EDIT: By that I mean the compressor only came on when the CCU called for cool, just like it should normally. The compressor did not run if A/C was not selected.

Sounds like everything was tried to kill the R4. Over chaging till it throws the belt. Too much oil etc.
